Job Description :
The Registered Nurse (RN) or Licensed Practical Nurse (LPN) provides assistance to the Nurse Manager with the management of the resident's health plan, supervising the day-to-day nursing activities and Caregivers during the shift in accordance with State & Federal regulations and facility policies and procedures. Promotes effective communication, continuous improvement,

Accountability, team building, conflict resolution and delegation. Relationships with residents,
family, residence and the community as a whole are emphasized.


 


Responsibilities:



  • Provides quality and safe delivery of resident care by implementing current standards of nursing practice.  Demonstrates competency in necessary clinical skills.

  • Reviews and implements the care plan to meet the residents physical, mental and psychosocial needs.  Delivers and monitors Residents care, based on ACHA 1823.  Administers medications and treatments as ordered by attending physician.  Communicates with all disciplines to ensure that care is delivered according to the resident care plan. 

  • Communicate clearly and directly with patients; provide emotional support to patients and family members under stress;

  • Gives and receives adequate shift report on residents at start and end of shift.  Ensures caregivers also give/receive report on resident.

  • Fill out and update patient documents and charts;

  • Encourage and support fellow team members to reach common goals;

  • Emotional support;

  • Place the needs of our clients first;

  • Team player;

  • Maintains safe and clean working environment by complying with procedures, rules, and regulations;

  • Ensure medical records are up to date;

  • Other duties as assigned by the Supervisory staff;

  • Utilizes the nursing process to assess, plan, implement and evaluate patient care;

  • Take vitals, including blood pressure, heart rate, temperature, etc;

  • Collect, track, and maintain patient history; record any new data;

  • Take instruction from and report updates to registered nurse/overseeing physician;

  • Report any unusual or drastic changes in patient behavior/symptoms; seek help from superiors when needed.


Required Qualifications :
Requirements:

  • Excellent interpersonal communication skills; great bedside manner;

  • Understanding of standard medical practices and procedures;

  • Ability to work some evenings, holidays, and weekends;

  • The ability to exercise initiative and independent judgment;

  • A Valid nursing license in the state of PA is required;

  • Bachelor’s and/or associate’s degree in nursing or related field;

  • Acceptance of philosophy, mission, and goals of Firely Services;

  • Licensed Practical Nurse ( LPN ) Current VA State License, Registered Nurse (RN) Current VA State license;

  • Current CPR Certification;

  • Current PPD or Chest X-Ray;

  • One year prior Licensed Practical Nurse (LPN) experience preferred;

  • Ability to Work Independently with access to RN Supervisor.
