Intake Specialist / Service Coordinator

One of our nation's leading homecare companies seeks a full time patient navigator to join their local office. This position will be a hybrid opportunity where a compassionate person can use their empathy, drive and ambition to serve our fragile senior community.  

 

The successful candidate will work directly with the owners of this organization to prequalify all of the new leads that come into the office via phone, in-person visits to hospitals, nursing centers and patients homes. During these visits you will discuss homecare services, the types of care a patient may need and the costs associated with that care. Once a client selects to utilize services you will act as the navigator as the client onboards by setting up the non-clinical plan of care, introducing the care givers, and following up on an ongoing basis.

 

Additionally, you will be asked to attend networking events, make sales calls to hospitals, nursing facilities, doctor offices and be the community advocate. Possibility of on-call on some weekends.

 

Excellent pay based on qualifications, benefits, reimbursement for mileage.
