Our home care hiring partner is seeking a community liaison for Collier County, FL. 




Position Objective:


 


       Our Community Liaisons are VITAL in assuring that we meet the needs and expectations of both our clients and our caregivers.  They also are a core essential element in our community-based marketing and education efforts. This position is mostly in the field.


 


 


Position Responsibilities:


 


Our Community Liaisons are engaged in the following activities:


 



  • Assessments – the initial meeting with potential new clients to develop a care plan identifying all expectations of care by the client and their family.

  • Community Awareness – Develop, foster, nurture, and enhance relationships with a variety of community resources that engage and interact with our Senior population that could benefit from the services we provide as well as be a resource for our clients, family members and circle of support. 

  • Community Education – Identify and facilitate opportunities for education, seminars, presentations, speaking engagements, events, and any other format that will enable an educational presentation on the needs of our Seniors, their specific challenges, and issues, and how Visiting Angels is dedicated to assisting those who are in need.

  • Customer Service - Conduct “Supervisory Visits” with current clients for quality assurance of care being provided.  Update Care Plan as appropriate for changing conditions or needs, and engage with family members, physicians, and auxiliary services for continuity of care purposes.  The consistent and reliable delivery of quality care is paramount to the company.



  • Caregiver Service - Engage with existing caregivers regarding their individual needs to deliver the consistent message that they are a highly valued member of the team.  During Supervisory Visits – ensure that they are meeting job requirements/policies and procedures.  Take corrective action as needed.

  • Caregiver Recruiting - Engage with various community resources where the recruiting of quality caregivers can be achieved.  Opportunities for recruiting relationships exist with various Faith organizations, specialized schools (RN, C.N.A, Medical Assistants, etc.), and many others.  Maintaining a consistent source of dedicated caregivers is essential to the delivery of care.

  • Assist in the “on-call” rotation for after-hours inquiries to assure prospects are contacted quickly.

  • All other duties as assigned.


           


                       


Operations and Procedures:


 


       The Community Liaison will follow all Policies and Procedures, including those in the Administrative Employee Caregiver Handbook and Staffing Guidelines.


 


                       


Supervision:


       The Community Liaison will report to the Operations Manager.
